MONIT-ERR^®

The MONIT-ERR^® service offers pharmaceutical companies the opportunity to monitor a specific drug product or groups of products for any occurrence of a medication error. Med-E.R.R.S. can monitor reports of potential and actual medication errors received through various error reporting mechanisms.  ISMP, the parent company of Med-E.R.R.S., has a formal agreement with the United States Pharmacopeia (USP) to receive all medication error reports from the USP-ISMP Medication Errors Reporting Program (MERP), a voluntary, confidential reporting program. In addition to sharing the data between ISMP and USP, as part of being FDA MEDWatch Partners, ISMP and USP also share the reports with FDA. In turn, through the Freedom of Information ACT (FOI), FDA shares requested error reports with ISMP.
