PROGRAM OVERVIEW
Since up to 25% of reported medication errors are related to issues with product packaging and or labels, it is important for pharmaceutical companies to develop them in a safe manner. Failure Mode and Effects Analysis (FMEA) and human factors are two methodologies that can be used to help increase the chances of designing a clear, unambiguous, easy-to-understand package or label.

OBJECTIVES
Following completion of this teleconference, participants should be able to:
- Identify and discuss label and packaging issues that lead to medication errors as identified by ISMP
- Describe newly established medical product design initiatives that are under way at the US FDA and within the pharmaceutical industry to help prevent errors
- Understand the role of Failure Mode and Effects Analysis (FMEA) in the detection and prevention of package and label-related medication errors
- Recognize that many error-prone drug labels arise from not following human factors engineering guidelines
- Understand the human factors engineering methods that are crucial for detecting issues and enhancing label and packaging design
- Determine that innovative packaging can improve safety and serve a central role in disease management

TELECONFERENCE POLICIES
Registration
- One telephone line connection is allowed per registration.
Others may listen on a telephone line extension; or a speakerphone
may be used for larger groups.
- You may not internally conference in other sites to our
teleconference.
- Each specific site must register individually.
- We cannot accept registrations without an email address
and the name of a contact person.
- Registration for this teleconference will end at midnight ET on Tuesday November 13th. CDs will be available
for purchase after the teleconference.
Cancellations
- Cancellations must be made via an email message to billing@med-errs.com.
- Cancellations received before the start (1:30 pm ET)
of the teleconference will receive a full refund or credit.
- Cancellations received after the start (1:30 pm ET)
of the teleconference and within 3 business days after the
teleconference will be assessed a cancellation fee of $50.00.
- If your organization does not cancel within three (3)
business days after the date of the teleconference and does
not participate, the full registration fee will be charged.

receive the teleconference materials that are sent out approximately
2 days before the teleconference via email, please send

prior to the start of the teleconference.
